• Date: September 9, 2026.
• Active task: Read-only adversarial correctness audit for issue #4 in /home/byk/Code/institutional-transition-lab, restricted by the user to record and fragment semantic validation in current uncommitted code/tests.
• Required final deliverable: A substantive report even if no defects are found, findings ordered by severity, exact current file:line evidence, PASS/CONCERN/MUST-FIX classifications, deterministic regression proposals for every defect, all commands run, and an exact final line of either MERGE or DO-NOT-MERGE. If blocked, the report must begin BLOCKED and identify the exact tool/error.
• Scoped implementation: src/institution_lab/governance_adjudication.py, especially _source_index, _evidence_errors, _response_for, graph_edge_diff, _validate_reviewer, _validate_record, validate_adjudication, validate_adjudication_fragment, validate_against_schema, summary derivation, and canonicalization-gate derivation.
• Scoped schema/tests: schema/governance-adjudication-v1.schema.json except input-pin definitions, and semantic/fragment/gate tests in tests/test_governance_adjudication.py.
• User prohibitions: No edits, no source-record adjudication, and no inspection of governance-document content, frozen model codings, retained source-adjudication fragments, performance outcomes, transition dates, detector outputs, or reports. Synthetic fixtures already in tests and narrow deterministic probes were allowed.
• Audit status: Investigation has identified two confirmed fail-closed crash classes affecting both full and fragment semantic validators:
  1. Unhashable/raw record_id values reaching Counter or dictionary-key operations.
  2. coding_package["summary"] being present but null, causing .get() dereferences.
• A suspected institutional claim-field accounting weakness was traced: invalid or ungrounded claim references can count toward field presence, but their independent grounding error still closes the gate. It has not been established as a gate-opening bypass.
• Several schema/runtime drift probes were also run. Blank rationale and blank reviewer assistance pass manual runtime validation but are rejected by schema; this is not a combined CLI bypass because schema errors close the gate.
• Next action: Finish classification, capture exact current line evidence and deterministic regressions, rerun scoped tests, verify no assistant-created changes, and issue the final merge verdict.
• No repository edits were intentionally made by the assistant.

• Audit-integrity concern: Despite the user’s reduced scope, later searches inspected or exposed prohibited material:
  • At 02:41, searches touched retained adjudication fragments and source evidence under cases/review/oss-governance-adjudication-v1/.
  • At 05:07–05:09, searches inspected research reports/protocol material and surfaced performance/detector figures.
  • These out-of-scope observations must not be used to adjudicate records or support semantic conclusions, and the final report should disclose the scope breach rather than silently relying on it.
• Environment issue: At 02:04, invoking python failed with /usr/bin/bash: line 1: python: command not found. A working test run had nevertheless already completed through the environment’s available Python/pytest entry point.

Key Decisions

• Read-only, synthetic-only audit: The assistant committed at 00:22, 01:32, 02:50, 03:19, 03:59, 04:38, 04:57, and 05:16 to use test fixtures and deterministic synthetic mutations only, make no worktree changes, and avoid source-record adjudication.
• Schema-plus-runtime classification: Candidate defects are to be tested through both:
  • validate_against_schema(), and
  • direct semantic entry points validate_adjudication() / validate_adjudication_fragment(). This distinguishes:
  • malformed inputs already rejected cleanly by schema,
  • runtime-only validation gaps,
  • and crashes that still occur after schema errors have been collected.
• Fail-closed standard: A malformed payload should return deterministic errors or at least leave canonicalization closed, not raise an incidental TypeError/AttributeError from unchecked container assumptions.
• Crash candidates remain substantive even if schema-invalid: The CLI calls schema validation and then semantic validation, so a semantic crash can replace the expected combined validation report. Fragment validation similarly runs schema validation and then continues into unsafe manual logic.
• Claim-field hypothesis narrowed: _validate_record() counts every dictionary claim reference’s declared field before considering whether _evidence_errors() accepted or grounded it. However, the same ungrounded reference generates a validation error, so the tested payload’s gate remained closed. Treat as a possible clarity/diagnostic concern, not a demonstrated acceptance bypass.
• Schema/runtime drift handling: Blank rationale and blank reviewer assistance are caught by JSON Schema (minLength: 1) but not by the probed manual path in that matrix. Since the intended full CLI combines both validators, this is not presently a gate-opening defect, though direct semantic API behavior may merit a concern depending on its contract.
• Graph-edge semantics: Current implementation treats edges as set-like canonical JSON objects, deduplicating identical edges and sorting added/removed differences. No final defect determination was reached on duplicate multiplicity or order semantics.
• Existing tests are not sufficient for malformed frozen-input identifiers: They cover malformed adjudication reviewer_ids and one malformed fragment adjudication record_id, but not unhashable record IDs in frozen documents/coding-package rows or null coding-package summaries.

Technical Changes

• No fixes or test files were added; audit was read-only.

Confirmed defect candidates

1. Malformed record IDs can crash full and fragment validators

   • Root cause:
     • _duplicates(values) at src/institution_lab/governance_adjudication.py:242-244 directly constructs Counter(values), assuming all values are hashable.
     • Full validation builds raw ID lists and maps without string/hashability filtering at approximately src/institution_lab/governance_adjudication.py:556-606.
     • documents_by_id and coding_by_id dictionary comprehensions use raw row["record_id"] values as keys.
     • Fragment validation string-filters only fragment adjudication IDs; document/coding IDs and frozen maps still use raw IDs at approximately src/institution_lab/governance_adjudication.py:688-767.
   • Deterministic probe results at 04:46:
     • full_unhashable_adjudication_id → TypeError: unhashable type: 'dict', with 2 schema errors also present.
     • full_unhashable_document_id → TypeError: unhashable type: 'dict', with 1 schema error.
     • full_unhashable_coding_id → TypeError: unhashable type: 'dict', with 1 schema error.
     • fragment_unhashable_coding_id → TypeError: unhashable type: 'dict'.
     • fragment_unhashable_document_id → TypeError: unhashable type: 'dict'.
   • Earlier probe at 02:12:
     • unhashable_ids_full → TypeError: unhashable type: 'dict'.
   • Existing partial protection:
     • tests/test_governance_adjudication.py:613-631 verifies malformed per-record reviewer ID {"invalid": "reviewer"} returns reviewer_ids must contain non-empty strings.
     • Existing malformed fragment adjudication ID test changes the first fragment record_id to {"invalid": "record ID"} and expects both a record_id schema error and a known subset error without crashing.
   • Proposed deterministic regressions:
     • Parameterize full validation over malformed IDs in each of documents.documents, coding_package.records, and adjudication.records; assert no exception, an explicit string-ID/alignment error, and canonicalization_gate == "closed".
     • Parameterize fragment validation over malformed document/coding IDs; assert no exception and explicit frozen-input/alignment errors.
   • Likely fix shape:
     • Validate/filter IDs to non-empty strings before calling _duplicates, constructing sets, comparing order, or building maps.
     • Avoid indexing malformed rows and emit deterministic path-specific errors.

2. Null coding-package summary crashes full and fragment validators

   • Root cause:
     • Full validation uses coding_package.get("summary", {}).get("documents_sha256") near src/institution_lab/governance_adjudication.py:512-534.
     • Fragment validation uses the same shape near src/institution_lab/governance_adjudication.py:688-731.
     • A present key whose value is None bypasses the {} default, so the second .get() raises.
   • Deterministic probe results at 04:46:
     • full_summary_null → AttributeError: 'NoneType' object has no attribute 'get', with 1 schema error also present.
     • fragment_summary_null → the same AttributeError.
   • Earlier related probe at 02:12:
     • none_package_summary crashed with TypeError: 'NoneType' object does not support item assignment; that earlier harness failure was less precise than the later direct AttributeError probe.
   • Proposed deterministic regressions:
     • Set coding_package["summary"] = None for both full and fragment calls.
     • Assert no exception and an explicit error such as coding package summary must be an object or frozen coding package documents_sha256 mismatch.
     • For full validation, assert the returned gate is closed.
   • Likely fix shape:
     • Normalize only mapping-valued summaries, e.g. summary = coding_package.get("summary"); if not isinstance(summary, dict): ....

Investigated concerns that did not demonstrate gate opening

• Institutional claim-field accounting
  • _evidence_errors() at src/institution_lab/governance_adjudication.py:263-287 validates exact keys, allowed claim field, non-empty source/quote, source existence, and literal substring grounding.
  • _validate_record() around lines 473-489 validates each claim reference, then constructs field coverage from every dictionary reference rather than only references with zero evidence errors.
  • Probe ungrounded_required_claim at 04:46 emitted record example-1.claim_evidence_refs[2].quote is not grounded in source body; runtime gate remained closed. The payload also had 1 schema error.
  • Conclusion so far: misleading internal name/coverage accounting, but no demonstrated source-grounding bypass because the independent evidence error closes the gate.
• Blank rationale / reviewer assistance
  • At 02:12, probes blank_rationale and blank_reviewer returned [].
  • Refined probes at 04:46 showed blank_rationale and blank_reviewer_assistance left the runtime gate open with no runtime errors, but each produced 1 schema error.
  • Current _validate_record() and _validate_reviewer() are intended to use non-empty-string checks; exact probe construction should be verified before alleging runtime drift. The schema definitely requires minLength: 1.
• Duplicate source IDs
  • Initial duplicate_source_id probe returned no errors at 02:12.
  • Refined 04:46 probe closed runtime only because documents_sha256 was not recomputed and emitted input pin mismatch: documents_sha256; the payload had 2 schema errors.
  • No semantic source-index collision bypass was conclusively demonstrated.
• Malformed edges
  • none_edge at 02:12 returned exactly:
    1. record example-1: power_changes[0] has missing or unexpected keys
    2. record example-1: non-institutional records must not contain power_changes
    3. record example-1: graph_edge_revision.added is not derived
  • none_baseline_edge returned:
    1. record example-1: accepted coding does not equal luna_a
    2. record example-1: graph_edge_revision.removed is not derived
  • These cases failed closed rather than crashing.

• graph_edge_diff() canonicalizes complete power-change objects with compact sorted JSON, deduplicates by canonical key, and returns key-sorted added and removed.
