Use durable per-document storage with unique (tenant_id, document_uri) and fields for tenant ID, document URI, content hash, title, URL, date, status, Loops campaign/message IDs, failure reason, and timestamps; rejected looser ad-hoc status tracking because retries and manual approval need an unambiguous lifecycle.
