PTY JSON envelopes

Chose JSON-only model-visible envelopes for PTY spawn, read, and exit results over XML-like control tags and interpolated trusted delimiters because terminal output is attacker-controlled and can imitate control messages. Keep operational instructions out of JSON fields; direct tools return data while event-driven completion supplies the exit envelope.
