JSON help sixel isolation

Trap: formatting ordinary branded help and converting it to JSON looks reusable, but it triggers printCustomHelp() terminal I/O and the Sixel probe for output that is discarded. Fix: --json must always use structured introspection directly; when no command path is supplied, it must never call the human formatter.
