Frozen source truncation

Trap: truncated tool output and evidence_bounds.source_text_truncated=true look interchangeable because both omit visible text. Fix: recover bounded chunks only when the frozen component itself is complete; when the envelope marks it truncated, never add later-recovered bytes or supplemental claims. If omitted text prevents grounding complete actors, rights, targets, and scopes, state the limitation and abstain.
