AI Protocol Endpoint Auth Framing

Chose separate Protocol, Endpoint, Auth, and Framing contracts over provider-specific monoliths because request semantics, URL construction, per-request authentication, and byte framing vary independently. Route defaults own headers, limits, generation, provider options, and HTTP policy; endpoint owns host/path/query, while protocols never import provider facades.
