validate_adjudication unhashable record_id

Trap: passing raw record_id values to _duplicates()/Counter and dictionary comprehensions looks safe because JSON Schema requires strings, but full and fragment validation continue after collecting schema errors; dict/list IDs then raise TypeError instead of returning a closed-gate report. Fix: validate and filter non-empty string IDs before duplicate, set, order, or map operations, and regress malformed IDs in document, coding-package, and adjudication rows.
