Fragment filename ordering

Trap: concatenating adjudication fragments in lexical filename order looks deterministic, but filenames encode reviewer subsets rather than authoritative corpus positions and can produce a false frozen-order failure. Fix: preserve each reviewer and record object exactly, reject conflicting duplicate reviewer IDs or duplicate records, order records by documents.json, and verify exact ID-set, count, uniqueness, abstentions, and object equality with standalone JSON checks.
