ci-poller cross-command ordering

Trap: comparing separate gh and node logs looks sufficient because each records the expected calls, but their indices cannot prove cross-command order. Fix: append both stubs to one event log and assert stale ci-ready removal occurs before final approval validation, which occurs before adding fresh ci-ready.
