ci-poller branch lookup fallback

Trap: falling back to the issue SHA when check-suite or branch-ref lookup fails looks safely conservative, but after a branch moves it checks stale CI and may add ci-ready during a transient API failure. Fix: exit only that issue’s poll cycle while retaining accepted and ci-pending; only an authoritative, unchanged branch HEAD may advance.
