Review implementation, tests, and workflows together

When working in this repository, first build a comprehensive, codebase-grounded understanding across the relevant source modules, CLI entry points, GitHub Actions workflows, and tests. Treat tests as executable specifications for exact behavior, including error messages, ordering, output formatting, security boundaries, immutable action pins, fail-closed validation, and edge cases. Trace changes through all interconnected approval, attestation, CI polling, revision, publishing, and issue-update paths rather than analyzing a file in isolation. Preserve existing invariants and verify proposed changes against both unit and workflow integration tests.
