Unused bindings use underscore

Prefix intentionally unused variables and parameters with _. Chose the underscore convention over ordinary unused names or lint suppressions because the configured no-unused-vars rule recognizes _ as intentional, while ordinary names fail validation and suppressions hide accidental dead code.
