Provider failure classification and capability learning

Trap: classifying a worker response containing an upstream provider error envelope as worker incapability looks reasonable because no usable answer was produced. Fix: classify it as upstream-error, because the worker successfully contacted the provider and returned its failure; incapability learning would incorrectly disable a functioning worker path.
