applyResponsesEvent final text authority

Chose completed response.output_text.done text as authoritative over retaining concatenated deltas because the provider’s final text is more reliable and may correct provisional fragments. In applyResponsesEvent(), assign item.text = text; still reject incompatible lifecycle, identity, or terminal structure rather than using final text to mask contradictions.
