Never return empty results

Never return an empty response, progress-only update, or context warning when a result is expected. For blocked or read-only research, return the substantive result available within the authorized scope; if a read fails, name the exact file and error. Extra investigation looks helpful but violates a frozen evidence boundary, while silence hides interrupted work.
