Warmup TTL savings exclusivity

Chose mutually exclusive warmup-hit and one-hour TTL savings over crediting both because both explain the same returning-turn cache read and double counting would inflate savings. Credit at most the smaller of returning cache-read tokens and the prefix refreshed by a paid warmup; otherwise record TTL savings only for gaps over five minutes.
