Trap: comparing a forwarded sparse output_index with its dense terminal position looks correct because rebuilt terminal output is compact, but direct principal frames preserve provider coordinates and may already be sparse. Fix: compare each already-forwarded source→public mapping before and after adding the newly hidden recall index; reject only mappings that change, so higher-index recalls succeed while lower-index recalls still fail closed.
